The PHS/EHD, Unit IV e a �t e€ti�istion due to surface <br /> and subsurface contamination as a result of unauthorized releases from both above <br /> k-j and below ground storage tanks As of this date, no work plan to investigate the <br /> ( contamination has been submitted to the Site Mitigation Unit,. Site is current on <br /> the UST Contaminated Site List. Contact Diane Hinson, Site Mitigation Unit, (209) <br /> 468-3452 for assessment information. <br /> 2. The well must be repaired under inspection by the Environmental Health Division <br /> (San Joaquin County Ordinance 1862, Section 5-4100) as follows: Seal domestic well <br /> (needs chlorinator plug and cualking). <br /> 3. Refuse, garbage and other solid waste material must be stored, removed and <br /> transported to an approved landfill in accordance with San Joaquin County <br /> Ordinance 2887, Section 5-2400. <br /> 4.
