A `11111, 11e D AR I N Environmental Health Department <br /> U4J ; I Y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> R&A MILLER INC 14125 E Comstock Rd, Linden August 27, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> A 1,000 gallon aboveground storage diesel and 12.5 gallons(approximately 25 pounds)of paraquat observed in the <br /> chemical storage container were found on site that have not been reported on the hazardous materials inventory. <br /> Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The site map did not address all required elements. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA". <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload a <br /> site map that addresses all required information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response plan had an incorrect facility address,was lacking the CUPA phone number(209) <br /> 468-3420 and did not identify the location of emergency response equipment.A fire extinguisher and spill kit were <br /> observed in the chemical storage container at the time of inspection. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> FA0022231 PR0538719 SCO01 08/27/2019 <br /> EHD 19-01 Rev.8/22/2019 Page 2 of 4 Hazardous Materials Program OIR <br /> 1868 E.
