SAN <br /> J D AR I N Environmental Health Department <br /> U4J ;� Y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> R&A MILLER INC 14125 E Comstock Rd, Linden August 27, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 10 HSC 25508.1(a)-(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. <br /> During the inspection, it was discovered that a 1,000 gallon aboveground storage tank(AST)containing diesel is <br /> present on site and the updated information has not been submitted.Within 30 days of one the following events, <br /> business shall electronically update the information submitted to the California Environmental Reporting System <br /> (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures,the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br /> plan readily available for review. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by September 26, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152). To schedule an <br /> appointment, please call (209)468-3420. <br /> Inspector provided: return to compliance certification, corrective action statement, copies of photos taken during <br /> inspection, official inspection report. <br /> Notes: Facility personnel stated at certain times of the year, the site may store up to 60 gallons of Intrepid. <br /> Ensure all reportable quantities of hazardous materials are reported into the chemical inventory within 30 days <br /> of being on site. Empty pesticide containers were observed on site.
