i. Verification shall be obtained from the Solid Waste Division (468-3066)that this development has <br /> met the requirements of the Waste Plan Format for Development Projects. Final Site Plan is <br /> required upon submittal of verification. <br /> j. Permit Registration Documents (PRD's)shall be filed with the State Water Resources Control <br /> Board (SWRCB). Waste Discharge Identification (WDID) Number issued by SWRCB shall be <br /> submitted to the Department of Public Works for file. Contact SWRCB at (916)341-5537 for <br /> further information. Coverage under the SWRCB General Construction Permit Order 2009-0009- <br /> DWO shall be maintained throughout the duration of all phases of the project. <br /> Informational Notes: <br /> (i.) This property is subject to the requirements of San Joaquin County Mosquito&Vector Control <br /> District(209-982-4675)and the California Health and Safety Code for the prevention of <br /> mosquitoes. Best Management Practices (BMP)guidelines for stormwater devices, ponds and <br /> wetlands are available. <br /> (ii.) All future building permits for projects located within a Special Flood Hazard Area at the time of <br /> permit issuance shall meet the San Joaquin County flood hazard reduction requirements(Title 9, <br /> Chapter 9-1605)and all requirements of the State of California (CCR Title 23)that are in force at <br /> the time of permit issuance. As an example, these requirements may include raising the finish <br /> floor elevation one foot above the expected flood level and/or using flood resistant materials. <br /> 4. ENVIRONMENTAL HEALTH DEPARTMENT(Contact Staff:Ted Tasiopoulous, [209]953-7698) <br /> a. An addendum to the soil suitability/nitrate loading study dated June 4, 1999 incorporating the <br /> proposed expansion of the existing religious assembly facility shall be submitted to the <br /> Environmental Health Department, indicating that the area is suitable for septic system usage. <br /> The studies must be approved by the Environmental Health Department prior to issuance of <br /> building permit(s). (San Joaquin County Development Title, Section 9-1105.2(d)). A review fee of <br /> $610 must be paid at time of submittal to the Environmental Health Department. <br /> The sewage disposal system shall comply with the on site sewage standards of San Joaquin <br /> County prior to approval. A percolation test that meets absorption rates of the manual of septic <br /> tank practice or E.P.A. Design Manual for onsite wastewater treatment and disposal system is <br /> required for each parcel. A permit fee of$122 per percolation hole is required. <br /> b. Construct a water well under permit and inspection of the Environmental Health Department prior <br /> to occupancy. Should the number of non-resident individuals exceed 24 for at least 60 days per <br /> year, or the number of service connections exceed four, a yearly permit to operate a public water <br /> system will be required by the Environmental Health Department(San Joaquin County <br /> Development Title, Section 9-1120.2 and 9-1115.9.). <br /> The supplier must possess adequate financial, managerial, and technical capability to assure <br /> delivery of pure, wholesome, and potable drinking water in accordance with San Joaquin County <br /> Development Title, Sections 9-1120.2 and 9-1115.9 and C.C.R., Title 22, and Health and Safety <br /> Code, Section 116525 through 116570. <br /> NOTE: <br /> Applicant shall contact Adrienne Ellsaesser, Lead Senior Registered Environmental Health <br /> Specialist, at(209)468-0343, to determine if the existing well can be permitted as a public water <br /> system prior to final occupancy. <br /> San Joaquin County PA-1000217/Singh <br /> Community Development Page 11 <br />
