u. The property may be required to form a new zone within County Service Area No. 41 prior to <br /> approval of the Final Map to provide for the operation, maintenance, and improvement of storm <br /> drainage and street lighting facilities. A new assessment for the zone shall be approved by the <br /> property owners prior to approval of the Final Map. The assessment for the new zone shall be <br /> based on the cost for maintenance, operation, system improvements, and any replacement program <br /> of facilities within CSA 41. The applicant shall pay for the cost to process the formation and <br /> assessment for the new zone. In addition, the applicant shall pay for the maintenance and operation <br /> of the system from the time the facilities are accepted by the District until the transferred area is <br /> placed on the assessment role of the District. A State National Pollutant Discharge Elimination System(NPDES)Construction Permit Notice of <br /> Intent(NOI) shall be filed with the State Water Resources Control Board if over one acre of total <br /> ground is disturbed. <br /> w. This subdivision, including streets, is subject to the Count/Stormwater Quality Control Criteria Plan, <br /> which requires treatment, filtration, or infiltration of the 85 percentile storm and post-construction <br /> Best Management Practices. <br /> x. A San Joaquin County Stormwater Permit is required prior to approval of the Final Map. <br /> y. A Stormwater Pollution Prevention Plan (SWPPP)will be submitted to Public Works prior to any site <br /> grading. <br /> z. Post-development stormwater discharge rates to Mormon Slough shall not exceed the pre- <br /> development rate. <br /> aa. A State Reclamation Board Permit shall be required for all work done within the banks of Mormon <br /> Slough. <br /> bb. Mormon Slough and a 25-foot-wide strip measured from the top of the bank to the north shall be <br /> denoted as Lot"A" on the subdivision map and dedicated to the San Joaquin County Flood Control <br /> and Water Conservation District. <br /> cc. A 20-foot-wide PUE and storm drain easement shall be part of Lot B and will be dedicated to the <br /> County at the time of the Final Map. <br /> 3. ENVIRONMENTAL HEALTH DEPARTMENT(Staff Contact: Michael Escotto, 468-3443) <br /> a. Connection to an approved public water supply system is required (Development Title Section 9- <br /> 1120.2). A letter from the water supplier will be required which states that they have the capacity to <br /> serve the development and that the system meets State standards prior to recording the final map <br /> (Development Title Section 9-1100.3). <br /> b. Written confirmation is required from Public Works that improvements have been constructed or <br /> financial arrangements have made for any improvements for public sewer required by the agency. In <br /> addition, written confirmation by Public Works that the agency has or will have the sewer capacity to <br /> serve the development it also required (Development Title Section 9-1100.3) <br /> NOTE: Prior to Final Map approval, the remaining home on proposed Lot 1 shall be connected to <br /> public sewer. <br /> c.
