LOctber 13, 2004 <br /> NOA Project Number: E04122A <br /> A review of the "Notification of Hazardous Waste Discharge" files within one-quarter mile of the <br /> subject property was conducted on October 11,2004. The following notices within one-quarter mile <br /> were found for the dates of 1995 through the present: <br /> Date Address Description <br /> 8/14/02 4040 E.Main St. Fuel release from UST <br /> 3/27/01 1030 S.Olive Ave. Release of bITBE to soil—tank removal <br /> 6/11/99 4075 E.Main St. Release of TPH-g and MTBE—tank removal <br /> 1/27/99 4075 E.Main St. Contamination from tank removal <br /> L1/8/97 4050 E.Section Ave. Auto fluids disposed into ground <br /> 10/16/00 South-bound 99 south of Charter Way Big rig overtumed into Mormon Slough <br /> L7/27/99 North-bound 99 south of Main St. Auto accident resulting in trailer rolling into dry creek bed <br /> 1999 Hwy.99 and Mormon Slough Diesel fuel spill to soil,approximately 100 gallons <br /> The UST cases have been discussed in section 3.1.2,above. The incident at 4050 East Section Avenue <br /> is not considered likely to have an impact on the subject property because it is hydrologically separated <br /> from the subject property by Mormon Slough. Though the contamination from the accidents on <br /> LHighway 99 has most likely been cleaned up years ago,there appears to be at high risk of accidents in <br /> the area. Copies of these permits were not available the day of the file review. <br /> 3.1.4 San Joaquin County,10cultural Commissioner <br /> The San Joaquin County Agricultural Commissioner's Office was not contacted regarding use of <br /> agricultural chemicals on the subject property because the subject property has not been actively farmed <br /> since 1979, according to Mr. Winchell's testimony to Mr.Jeffrey. <br /> 3.1.5 [dater Supply and Sewage Disposal System <br /> Mr.Jeffrey indicated that the residence at 3740 East Guernsey Avenue receives municipal water and <br /> Lsewer services,while the residence at 3800 East Guernsey receives City water but has a private septic <br /> system. A well or pump at the rear of the property was abandoned years ago, he stated (personal l' <br /> communication, October 13,2004). <br /> The EHD well and septic permit files were searched on October 11,2004 in an effort to obtain copies <br /> of permits for any water wells or septic tanks on the subject property. One septic permit dated May 17, <br /> L1963 and one datedJune 14, 1971 were found for the subject property address of 3800 East Guernsey <br /> Avenue. On the first pemnit, the contractor or EHD staff noted "this system does not meet our �3•J <br /> requirements but owner agrees we will not be responsible if system fails." The second permit was for <br /> Ethe replacement of the septic tank. One septic permit dated December 29, 1994 was found for the <br /> ` 7 <br /> I <br /> "004 L <br /> L <br />
