An Environmental <br /> L Questionnaire and Disclosure Statement was completed by Mr. Zeiter on April 26, 2004 for use (,,z <br /> by NOA for this report. A copy of the responses has been included in this report as Appendix <br /> E. <br /> In his responses to the questionnaire, Mr. Zeiter indicated that the owner of the property since <br /> December 2002 has been Affordable Luxury Homes, and that the previous owner was Mr.J.C. <br /> ►� Dillard. He stated that the property is currently bare land. Mr.Zeiter was interviewed in person <br /> at nine o'clock on April 27, 2004. During this interview,he stated that the subject property has F <br /> been bare land since the 2002 purchase. <br /> Ms. Lorene Dillard was interviewed by telephone at nine fifteen on April 27, 2004. Ms. Dillard <br /> stated that she and her husband purchased the property approximately 30 years ago. She stated <br /> that the subject property had been bare land for the extent of their ownership and that she and <br /> Mr. Dillard had never used the property for any purpose. Ms. Dillard indicated that a trailer <br /> existed on the subject property at the time of their purchase but that her husband had <br /> immediately moved it off the site. She stated that,to the best of her knowledge,no underground <br /> or above-ground storage tanks had been located on the property, and no chemical mixing had <br /> ■- taken place on the property. <br /> 3.0 PAST AND CURRENT SOURCES OF CONTAMINATION <br /> 3.1 Research Methods <br /> NOA employs a standard practice for review of database information. On a street map showing <br /> the location of the subject property, a circle is drawn one-quarter mile out from the property <br /> boundaries, and street maps or web-based mapping services are then used to determine address <br /> ranges for all streets within the quarter mile. All agency databases searchable by address are then <br /> searched for these address ranges. <br /> 5 <br />
