Submit an Environmental Health Department(EHD)well permit application and$89 permit fee. <br /> Once the well permit has been approved,notify EHD of the proposed schedule for the work. <br /> 2. CPT1 log shows sandy silt,sand,silty sand and gravelly sand from 10 feet to 45 feet below surface <br /> grade(bsg)and the highest concentrations of contaminants in the soil evidenced in soil from SBI and <br /> SB2 are from 15 to 45 feet bsg. The vapor extraction wells screens should extend from 15 to 45 <br /> feet bsg. <br /> 3. The work plan didn't include all the well construction details.
