Project Description —Construction &Demolition/Inert Debris Facility Report <br /> Tracy Material Recovery Facility and Transfer Stah'on <br /> 30703 S. MacArthur Drive, Tracy, California 95377 <br /> Special Occurrences [14 CCR 17414 (d)] <br /> Tracy MRF & TS will maintain a daily log book or file of special occurrences <br /> encountered during operations and methods used to resolve problems arising <br /> from such events, including details of all incidents that required implementing <br /> emergency procedures. Special occurrences shall include, but are not limited to: <br /> fires, injury and property damage, accidents, explosions, receipt or rejection of <br /> prohibited wastes, lack of sufficient number of personnel pursuant to section <br /> 17410.2, flooding, earthquake damage and other unusual occurrences. In <br /> addition, Tracy MRF & TS will notify the LEA by telephone within 24 hours of all <br /> incidents requiring the implementation of emergency procedures, unless the LEA <br /> determines that a less immediate form of notification will be sufficient to protect <br /> public health and safety and the environment. <br /> Tracy MRF & TS will record and retain records of any serious injury to the public <br /> occurring on-site and any complaint of adverse health effects to the public <br /> attributed to operations. Serious injury is defined as any injury that requires <br /> hospitalization of a period in excess of 24 hours, or one in which a member of the <br /> public suffers permanent disfigurement or the loss of any member of their body. <br /> This information is kept on file in the administrative office under the supervision <br /> of the Site Manager. <br /> Complaint Procedures [14 CCR 17414 (e)] <br /> Tracy MRF & TS will maintain records of any written complaints received and <br /> actions taken in their regard. Tracy MRF & TS will record any public complaints <br /> received by the operator, including: <br /> • the nature of the complaint, <br /> • the date the complaint was received, <br /> • if available, the name, address, and telephone number of the person or <br /> persons making the complaint, and <br /> • any actions taken to respond to the complaint. <br /> This information is kept on file in the administrative office under the supervision <br /> of the Site Manager. <br /> Enforcement Agency Documentation [14 CCR 17414 (f)] <br /> Any approvals, determinations or other requirements of the LEA provided to <br /> Tracy MRF & TS will be kept on file in the administrative office under the <br /> supervision of the Site Manager. <br /> 25 170.18 CDI FacReport 121907.doc <br />
