f. The Traffic Impact Mitigation Fee will be required for this development. The fee is due and payable at the time of excavation plan approval. The fee currently is $151.36 per additional daily trip-end in the Lockeford area. The fee shall be automatically adjusted July 1 of each year by the Engineering Construction Cost Index as published by the Engineering News Record. The number of trip-ends may be determined by a proportion to the length in time of the excavation operation. (Resolution R- 96-592) g. An encroachment permit shall be required for work within the road right-of-way. (Development Title Section 9-1145.4) 3. PUBLIC HEALTH SERVICES (Staff Contact: Ted Norgard, 468-3443) a. No excavation is permitted into the groundwater table without specific approval from the Regional Water Quality Control Board, Central Valley Region (California Water Code, Chapter 4, Article 3, Section 13240). b. The materials from an excavation shall not be deposited into any watercourse nor in any way contribute to the pollution thereof (California Water Code, Section 13376). c. An approved potable water supply must be provided for all employees (Development Title Section 9-1100.1). d. Approved sanitary facilities must be provided for all employees (Development Title Section 9- 1100.1). 4. CALTRANS (Staff Contact: Chris Sayre, 948-7142) a. Traffic Impact Fees shall be collected on a "fair share" basis for future improvements at State Route 88 and Mackville Road interchange. 5. PACIFIC GAS & ELECTRIC a. PG&E owns and maintains a 12 kV/60kV pole line along the northwesterly side of the subject parcel. Access to the poles shall be maintained during the quarry excavation. The cost of any relocation of this pole line shall be at the permit applicant's expense. b. No excavations will be allowed within fifty feet of the centerline of said pole line facilities or the anchor guy facilities. Conditions of Approval - 6 - QX-99-2
