Addendum to Phase I ESA Report dated 28 May 2003 <br /> Land Use Application #PA-03-346 (MS) SR#0034509 <br /> 1859 East McAllen Road, Stockton, California <br /> 1.0. INTRODUCTION <br /> At the request of Mr. Michael F. Escotto of the San Joaquin County Environmental Health <br /> Department (SJCEHD),Advanced GeoEnvironmcntal, Inc. (AGE)has prepared this addendum to <br /> our report,Phase I Environmental Site Assessment,APNs 092120-05, 06 and 140-01, 02, Stockton <br /> California and dated 28 May 2003. <br /> Theobjective of this report is to resolve five insufficiency issues noted in your letter,RE: Surface <br /> and Subsurface Contamination ReportforLand Use Application#PA-03-346(MS),Service Request <br /> #SR0034509, dated 26 August 2003 and received in this office on 08 September 2003. Each action <br /> item will be discussed and presented with the agreed-upon resolutions as shown below. This <br /> addendum references 1859 East McAllen Road,Stockton and includes the additional site inspection <br /> of two parcels not included in the original Phase 1. <br /> Your attached fortes, Surface and Subsurface Contamination Report Requirement Checklist and <br /> Requirements Checklist Comments, 1859 East McAllen Road, Stockton, report date 5/28/2003, are <br /> used as the base for this report. A copy of your letter and all required documents are provided in <br /> Appendix A. <br /> 2.0. OVERVIEW <br /> A Phase I Environmental Site Assessment(ESA)was prepared for Mr. Scott Rishwain on 28 May <br /> 2003; the scope of the Phase I included San Joaquin County Assessor's Office Parcel Numbers <br /> (APNs)092-120-05 and 06 and 092-140-01 and 02.At the time of the Phase I,Parcel Map 092-140 <br /> showed Parcel 01 extending to East McAllen Road and Parcel 02 at the northeast comer of <br /> Parcel 01. <br /> Mr. Scott Rishwain,upon agreement with your office, submitted a xerox copy of our Phase I report <br /> in lieu of Surface and Subsurface Contamination Report as part of his Land Use Application(LUA) <br /> #PA-03-346 to split off the southern-most 20 acres of the Phase I Parcel 01. The Non-Vesting <br /> Tentative Parcel Map provided with the LUA differed from the scope of the Phase I report in that <br /> two small parcels located between McAllen Road and the Calaveras River were added to complete <br /> the Sperry Family holdings (APNs 092-140-12 and 096-050-13) and the southern portion of the <br /> Phase I Parcel 01 was shown as a new Parcel 02. The remainder of the Phase I Parcels 01 and 02 <br /> were shown as a new Parcel 01. <br /> A copy ofthe LUA Parcel Map is provided as Figure 1 and a copy ofAssessor's Parcel Map 096-050 <br /> Advanced Ge Eovlronmeatah tne. <br />
