6.0 SITE DOCUMENTS <br /> 6.1 Environmental Health Department Records <br /> The EHD was contacted regarding the Site address of 29467 South Lehman Road. <br /> According to Evette Johnson, no records related to environmental issues are on file for <br /> the Site (personal communication, April 14, 2009). <br /> Live Oak requested copies of the EHD well and septic permit files for the Site address. <br /> The only record on file was a 2003 permit for pump replacement at a domestic well. Mr. <br /> Robertson indicated, however, that there are no wells on the Site, this permit may refer <br /> to an adjacent property. It has been included in Appendix 4. <br /> 7.0 GOVERNMENT AGENCY RESEARCH <br /> 7.1 EPA Databases <br /> The California Environmental Protection Agency (EPA) manages contaminated sites <br /> through two sub-agencies: the State Water Resources Control Board (SWRCB) and the <br /> Department of Toxic Substances Control (DTSC). The SWRCB's GeoTracker database <br /> includes underground storage tank (UST) sites, leaking underground storage tank <br /> (LUST) sites, cleanup program sites, and land disposal sites. DTSC's EnviroStor <br /> database includes Federal Superfund sites, State response sites, voluntary cleanup <br /> sites, school cleanup sites, corrective action sites, and permitted hazardous waste <br /> facilities. <br /> Live Oak searched both databases on April 11, 2010 for any listed sites within one- <br /> quarter mile of the Site. The subject Site was not listed on either database, and no <br /> facilities were identified within one-quarter mile. <br /> 7.2 Environmental Health Department Databases <br /> On April 20, 2010, Live Oak reviewed two databases maintained by the EHD, the <br /> Underground Storage Tank Site Mitigation List, dated February 18, 2009, and the Unit <br /> IV Sites (LOP) list, dated November 10, 2009, for surrounding facilities of concern. No <br /> listed facilities were identified within one-quarter mile of the subject Site. <br /> Live Oak reviewed the Water Well Data Table dated March 10, 2009 for nearby <br /> addresses that have been tested for nitrate and dibromochloropropane (DBCP), <br /> common ground water contaminants in San Joaquin County. Seven nearby wells were <br /> identified at addresses located within approximately one mile of the Site. None of the <br /> wells had been tested for DBCP. Detected concentrations of nitrate as NO3 within <br /> these wells ranged from 20 to 135 ppm. Five of the wells had nitrate levels at 45 or <br /> LOGE0916 Page 4 <br />
