Attachment O <br /> Contingency Plan for Well Failures <br /> 1. Evidence of failure of the well to restrain the injectate to <br /> the tubing string and injection formations shall be orally <br /> reported to the Director within 24 hours. Subsequent <br /> reporting requirements shall be managed as detailed in Part <br /> II, Section D, Item 3 of this permit. The evidence of well <br /> failures could be observed in several different ways. The <br /> three main observations would be: (1) an increase in the <br /> tubing/casing annular pressure, indicating a leak in the <br /> tubing or packer; (2) a decrease in injection pressure for <br /> no apparent reason, indicating failure of the well casing <br /> below the packer; or (3) the worst case scenario, injectate <br /> at the surface indicating failure of the surface pipe and <br /> degradation of the cement placed around the surface pipe. <br /> Proposed action for Type 1 failure, annular pressure increase: <br /> - Cease injection well operation. <br /> - Pull tubing string and visually inspect for holes or <br /> splits in tubing. <br /> - Inspect packer and repair or replace if tool failure is <br /> indicated. <br /> - Hydrotest tubing to 5000 psi. Replace all tubing joints <br /> which fail when tested. <br /> - Replace packer and tubing in wellbore and resume injec- <br /> tion operations. <br /> Proposed action for Type 2 failure, injection pressure reduction: <br /> - Cease injection well operation. <br /> - Pull tubing and packer from well. <br /> - Run casing inspection log to determine point of casing <br /> failure. <br /> - Patch and repair casing over damaged interval . <br /> - Replace packer and tubing in wellbore and resume injec- <br /> tion operations. <br /> - Monitor well to observe successful repair job. <br /> Proposed action for Type 3 failure, injectate at the surface: <br /> - Cease injection well operation. <br /> - Pull tubing and packer from well. <br /> - Run inspection logs to determine point of failure. <br /> - Remedy failure by pumping cement through casing to fill <br /> void spaces which are allowing injectate to migrate to <br /> the surface. <br /> - Pressure test casing to insure integrity of repair job. <br /> - Replace packer and tubing in wellbore and resume injec- <br /> tion operations. <br /> 27 <br />
