The following is an itemized list of hazardous waste violations that have not been <br /> addressed for D & H AUTO TECH as of November28, 2018. <br /> Open violations from August 22, 2018 inspection <br /> Violation#403-Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> At the time of inspection, the following containers were found open on the west side of the shop while waste was not <br /> actively being added or removed from the containers: <br /> -5-gallon bucket of used absorbent <br /> -55-gallon drum of used antifreeze <br /> All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately <br /> close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Violation#601 -Stored hazardous waste onsite greater than 180 days. <br /> Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste <br /> stored on site at any time may store waste on site up to 180 days. The 5-gallon bucket of used absorbent lacked an <br /> accumulation start date. Also, at the time of inspection, Pha Cham, a mechanic, stated that the used oil in the drum <br /> beneath the oil filter crusher and in the drum beneath the oil filter drain, is left to collect in the drums until the used <br /> oil is picked up by the disposal company. The drum beneath the oil filter crusher and the drum beneath the oil filter <br /> drain lacked accumulation start dates. Since it cannot be determined how long the hazardous waste has been on <br /> site, immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest. Submit a copy <br /> of the manifest to the EHD within 30 days. <br /> Violation #606-Failed to completely label containers or portable tanks of hazardous waste. <br /> The 5-gallon bucket of used absorbent, the drum beneath the oil filter crusher, and the drum beneath the oil filter <br /> drain lacked all of the required labeling information. All hazardous waste containers shall be marked with the <br /> following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information. Since it cannot be determined how long these containers have been on site, immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the <br /> EHD. <br /> Violation#608 -Failed to properly manage used oil and fuel filters. <br /> Bills of lading for the recycling of used oil filters for the last three years were not found on site. A copy of each bill of <br /> lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading for used oil filters <br /> for the last three years, maintain them on site, and submit copies to the EHD. <br /> The 55-gallon drum of used oil filters lacked an accumulation start date. Facilities that generate up to one ton of <br /> used oil filters per year may store the filters up to one year. Facilities that generate one ton or more of used oil <br /> filters may store the filters for up to 180 days. Since it Cannot be determined how long these filters have been on <br /> site, immediately recycle these filters under a bill of lading and submit a copy of the bill of lading to the EHD. <br /> The 55-gallon drum of used oil filters was found with an unsecured lid and lacking an accumulation start date. Used <br /> oil filters shall be drained of free-flowing oil, stored in a closed, rainproof container, labeled as"drained used oil <br /> filters," and marked with an accumulation start date. Immediately properly close the container, and since it cannot <br /> be determined how long these filters have been on site, immediately recycle these filters under a bill of lading and <br /> submit a copy of the bill of lading to the EHD. Ensure that all used oil filters are managed in this way. <br /> Open violations from December 12,2014 inspection <br /> Page 2 of 3 <br />
