6.0 SITE DOCUMENTS <br /> 6.1 Environmental Health Department Records <br /> The EHD was contacted regarding the Site addresses of 19238 and 19242 East <br /> Highway 120. According to Evette Johnson, no environmental records are on file for <br /> these addresses (personal communication, November 18, 2010). <br /> Live Oak requested copies of the EHD well and septic permit files for the Site <br /> addresses. The following permits were found: <br /> • December 1964 permit for addition of one 50-foot leach line and one six-foot <br /> diameter by 10-foot deep seepage pit to residential system at 19238 E. Highway <br /> 120 (eastern residence). <br /> • August 1977 permit for pump repair at well at 19242 E. Highway 120. <br /> • September 1977 permit for new irrigation well at 19242 E. Highway 120. <br /> • July 1986 permit for destruction and replacement of domestic well at <br /> 19238/19242 E. Highway 120. <br /> Copies of the EHD records have been included in Appendix 4 of this report. <br /> r 7.0 GOVERNMENT AGENCY RESEARCH <br /> 7.1 EPA Databases <br /> The California Environmental Protection Agency (EPA) manages contaminated sites <br /> through two sub-agencies: the State Water Resources Control Board (SWRCB) and the <br /> Department of Toxic Substances Control (DTSC). The SWRCB's GeoTracker database <br /> includes underground storage tank (UST) sites, leaking underground storage tank <br /> (LUST) sites, cleanup program sites, and land disposal sites. DTSC's EnviroStor <br /> database includes Federal Superfund sites, State response sites, voluntary cleanup <br /> sites, school cleanup sites, corrective action sites, and permitted hazardous waste <br /> facilities. <br /> Live Oak searched both databases on November 26, 2010 for any listed sites within <br /> one-quarter mile of the Site. The subject Site was not listed on either database, and no <br /> listed facilities were identified within one-quarter mile. <br /> 7.2 Environmental Health Department Databases <br /> On November 19, 2010, Live Oak reviewed two databases maintained by the EHD, the <br /> Underground Storage Tank Site Mitigation List, dated May 7, 2010, and the Unit IV <br /> LOGE1028 Page 4 <br />
