This form must be completed and submitted with your other permit <br /> documents. The city department with which you are working will forward the form to our Office. <br /> Accurate completion of this survey form will allow us to ensure that you are in compliance with <br /> Chapter 6.95 of the California Health and Safety Code while you handle hazardous materials. <br /> The intent of this law is to increase business preparedness for einergencies,-help safeguard public <br /> safety agencies that might respond to a hazardous materials accident at a business, and to meet <br /> community-right-to-know objectives. Requirements for businesses include preparing an <br /> emergency plan for use by their employees in an emergency, training employees in emergency <br /> functions, and providing periodic updates of plans and inventories to emergency responders. <br /> In answering the survey form most businesses have three key questions: <br /> What is a "hazardous material"? <br /> The legislation that created this program established a very broad definition of a hazardous <br /> material. The law defines a "hazardous material" as any material that, because of its quantity, <br /> concentration, or physical or chemical characteristics, poses a significant present or potential <br /> hazard to human health and safety or fo the environment. This includes many common materials <br /> such as gasoline, oil, or antifreeze as well as industrial products such as pesticides, chlorine, and <br /> ammonia. Generally, if the manufacturer of the product that you use provides a Material Safety <br /> Data Shect (MSDS) for it, then it is defined as a hazardous material. if you arc, unsure whether a <br /> material you use is a hazardous material, then call our office at (209) 468-3969 for clarification. <br /> How much of a hazardous material must I have to fall undo program <br /> requirements? <br /> The general rule is that if you use 55 gallons of a hazardous liquid, 500 pounds of a hazardous <br /> solid, or 200 cubic feet of a hazardous gas at any one time in the year, then you must comply <br /> with this program. However, there are hazardous materials that because of their characteristics are <br /> tegulated at lower or higher quantities. GenBrally if you'report that you handle a hazardous <br /> material, but under the above standard program quantities, our office will contact you to verify <br /> your exemption. We will at the same time help identify materials that may deed to be reported at <br /> lesser or greater quantities than the standard rule shown above. <br /> How complicated is it to comply with this program? <br /> Our office will provide you with forms and fret assistance Cor completing the emergency plan and <br /> inventory as well as for documenting required employee trw;ing. <br /> 12/96 <br />
