(3 <br /> 1.3.2 Site Specific Permit History <br /> _ No permits were found during a recent file review at the EHD for the subject property address <br /> of 5400 West Highway 12. <br /> 1.3.3 Discussion of On-Site Septic Tank Repair <br /> No permit or repair records were found during a recent file review at the EHD for the subject <br /> property address of 5400 West Highway 12. l �' <br /> 1.3.4 Existing Septic Tank Usage in the Vicinity <br /> In an attempt to determine the septic system density for the vicinity of the Site, our office <br /> reviewed a recent aerial photograph of the subject property available from GoogleEarthT"'. We <br /> counted the building clusters within the industrial park the Site parcel is contained within, and <br /> made the assumption that each building cluster has one septic system. Using this method, we <br /> determined that there is an average of one septic system per acre in the immediate vicinity of <br /> the Site. <br /> _ The general vicinity consists of agricultural land to the north and south, and industrial <br /> development to the east. Agricultural land and the Flag City travel area are to the west. <br /> Interstate Route 5 is farther west. <br /> 1.3.5 Septic Permit History in the Vicinity ; S; <br /> Septic permits were requested for the properties adjacent to the site through EHD. Permits for <br /> the surrounding parcels were reviewed as follows: <br /> -- • 5136 West Highway 12, adjacent east, has a permit from 2010 for a new septic system <br /> installation; <br /> • 5100 West Highway 12, two parcels east of the site, has a permit from 1994 for a new <br /> septic system installation, a permit from 1996 for the installation of a sand-oil separator, <br /> permits from 1999 and 2003 for percolation tests, and a permit from 2004 for a new <br /> septic system installation for; <br /> • 4988 West Highway 12, three parcels east, has a permit from 1996 for a the addition of <br /> 160 feet of leach line and a sump; <br /> "'04 <br /> ©2015 Neil O.Anderson&Associates,Inc. <br /> A Terracon Company <br />
