3 ENVIRONMENTAL HEALTH DEPARTMENT(Staff Contact: Michael Escotto, 468-3443) <br /> a. An addendum to the soil suitability/nitrate loading study dated August 31, 2006, incorporating all <br /> phases of the proposed expansion shall be submitted to Environmental Health indicating that the <br /> area is suitable for septic usage. The studies must be approved by Environmental Health prior to <br /> issuance of building permits (Development Title Section 9-1105.2[d]) A review fee of$575.00 must <br /> be paid at the time of submittal to Environmental Health. <br /> The sewage disposal system shall comply with the on-site standards of San Joaquin County prior to <br /> approval. A percolation test that meets absorption rates of the manual of septic tank practice or the <br /> EPA Design Manual for on-site waste water treatment and disposal systems is required for each <br /> parcel. A permit fee of$115 00 per percolation hole is required. <br /> b. Submit to Environmental Health revised site plans showing the maximum number of employees and <br /> customers or parishioners the sewage disposal system is being designed for. In addition, show on <br /> revised plans that the leach field area will be barricaded so that it cannot be driven over, parked on, <br /> or used as a'storage area. This leach field area must be used for that specific purpose only, and it <br /> cannot contain any underground utility lines (Development Title Section 9-1110.4[c][5]). <br /> In addition, the revised site plan shall incorporate the 100 percent designed sewage disposal <br /> replacement area. <br /> c. Construction of an individual sewage disposal system under permit and inspection by Environmental <br /> Health is required at the time of development based on the Soil Suitability/Nitrate Loading Study <br /> findings (Development Title Sections 9-1110.3 and 9-1110.4) <br /> d. Construct a water well under permit and inspection by Environmental Health prior to occupancy. <br /> Should the number of nonresident individuals exceed 24 for at least 60 days per year, or the number <br /> of service connections exceed four, a yearly permit to operate a public water system will be required <br /> by Environmental Health (Development Title Sections 9-1120.2 and 9-1115.9). <br /> The supplier must possess adequate financial, managerial, and technical capability to ensure the <br /> delivery of pure, wholesome, and potable drinking water in accordance with Development Title <br /> Sections 9-1120.2 and 9-1115.9, CCR, Title 22, and Health and Safety Code Sections 116525 <br /> through 116570. <br /> NOTE: The applicant shall contact Adrienne Ellsaesser, Lead Senior Registered Environmental <br /> Health Specialist, at (209) 468-0343 to determine if the existing well can be permitted as a public <br /> water system prior to final occupancy. <br /> 4 SAN JOAQIN COUNCIL OF GOVERNMENTS (Staff Contact: Anne-Marie Poggio, 468-3913) <br /> a. This project is subject to the San Joaquin County Multi-Species Habitat Conservation and Open <br /> Space Plan (SJMSCP) and is located within the unmapped land use area. Per requirements of the <br /> SJMSCP, unmapped projects are subject to case-by-case review. This can be a 90-day process, <br /> and it is recommended that the project applicant contact SJMSCP staff as early as possible. After <br /> this project is approved by the Habitat Technical Advisory Committee and the SJCOG Board, the <br /> following process must occur to participate in the SJMSCP: <br /> (1) Schedule a SJMSCP Biologist to perform a pre-construction survey prior to any ground <br /> disturbance. <br /> Conditions of Approval - 4 - PA-0900291 <br />
