The fee for this permit is an annual fee of $450. <br /> 8. A monitoring program shall be implemented for the proposed Civil Engineered <br /> Septic System to validate the operation of the system as proposed by your <br /> Nitrate Loading Study. A monitoring program shall include routine quarterly <br /> sampling of sewage effluent to assess efficiency and denitrification process. <br /> Quarterly sampling of effluent prior to disposal into leach field shall include the <br /> following: <br /> (a) B.O.D. (b) T.S.S_ (c) T.D.S. (d) C.B.O.D. <br /> i <br /> 9. Monitoring ports shall be installed in leach field and shall be used for testing, <br /> starting 6 months after operation. Testing requirements for sampling this leach <br /> bed area include the following: <br /> (a) Sodium (b) Nitrate (c) TKN (d) TDS <br /> It is the responsibility of Musco f=amily Olive Company facility to employ a state certified <br /> lab, obtain, test, and provide results to the EHD for the above monitoring conditions, <br /> and submit this testing in a report to the EHD quarterly. <br /> Please address the above noted items by either submitting revised engineered plans (a <br /> review fee of $178.00 is required upon submittal) or by making the appropriate changes <br /> to the septic system. The EHD will re-inspect the system after the corrections/revisions <br /> are made. Please provide a minimum of 24 hours notice prior.to next inspection.
