'w Page# : 11 <br /> "ERRI & SON Permit# : 39-99-3900212 <br /> CONDITION - T <br /> ALDICARB (TEMIK) <br /> - ----------------------------- <br /> r-1 . The restricted materials permit, issued in the permittee ' s home county, <br /> shall indicate the amount of aldicarb the permittee has in inventory when <br /> the permit is issued. The inventory total shall include all aldicarb <br /> storage sites regardless of location. These storage site location (s) shall <br /> be noted on the permit . <br /> _ Aldicarb currently in inventory lbs . <br /> Storage site locations : (1) <br /> (2) <br /> (3) <br /> 2 . All subsequent restricted materials permit (s) shall identify the home <br /> county responsible for tracking aldicarb purchases and returns . <br /> Home county is : <br /> 11-3 . The permittee shall report each purchase and return of aldicarb to the home <br /> county in which they are headquartered. The report shall include the <br /> name (s) and addresses of the dealer (s) and the total amount (s) of aldicarb <br /> purchased or returned; and the delivery address . The report shall be made <br /> within 10 days of the purchase and may be verbal or written. The county <br /> agricultural commissioner shall maintain these reports with the permit . <br /> -4 . A recommendation, written by the property operator or a licensed pest <br /> control advisor, must be in the possession of the pesticide applicator at <br /> the use site . <br /> 5 . The permittee shall keep all aldicarb sales invoices and recommendations <br /> with the associated restricted materials permit .
