i <br /> 1 <br /> (2) All waste tires (whole and/or tire equivalents) <br /> 2 <br /> shall be removed by July 30, 1999 . Copies of <br /> 3 <br /> destination receipts and waste tire manifests must be <br /> 4 <br /> submitted to the CIWMB on or before August 7, 1999 . <br /> S <br /> B . Operation Plan <br /> 6 <br /> (1) During the waste tire removal operation commencing <br /> 7 <br /> forthwith, adequate fire prevention measures must be <br /> 8 <br /> provided. At a minimum, the following equipment shall <br /> 9 _ <br /> be on site to aid in the control of fires : <br /> 10 <br /> * 1 dry chemical fire extinguisher <br /> 11 <br /> * 1 two and one-half gallon water extinguisher <br /> 12 <br /> * 1 pike pole at least ten feet in length <br /> 13 <br /> * 1 round point and 1 square point shovel <br /> 14 <br /> (2) During the waste tire removal operation commencing <br /> 15 <br /> forthwith, accessible fire lanes between waste tire <br /> 16 <br /> piles and structures shall be kept free of flammable or <br /> 17 <br /> combustible material and vegetation. Access to fire <br /> 18 <br /> lanes for emergency vehicles must be unobstructed at . <br /> 19 <br /> all times . <br /> 20 <br /> (3 ) During the waste tire removal operation commencing <br /> 21 <br /> forthwith, the owner/operator shall ensure that open <br /> 22 <br /> flames, blow torches, or highly flammable materials, <br /> 23 <br /> including but not limited to, tire inner tubes , are not <br /> 24 <br /> stored within 40 feet of any waste tire pile . <br /> 25 <br /> (4) During the waste tire removal operation commencing <br /> 26 <br /> forthwith, treatments or methods necessary to prevent <br /> 27 <br /> or eliminate vector breeding shall be undertaken, <br /> 4 . <br />
