S A N I U Q U U ( N Environmental Health Department <br /> Time In: 10:25 am <br /> Time Out: 11:00 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: JALOS TAQUERIA Date: 08/16/2019 <br /> Address: 25456 S SCHULTE RD,TRACY 95377 <br /> Owner/Operator: REYNOSO, HUGO ALBERTO Telephone: (209) 830-8066 <br /> Program Element: 1600 - FOOD PROGRAM Complaint#: C00050257 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> 000050257 states: "COMPLAINANT WAS AT FACILITY ON 8/5/19 ATAPPROXIMATELY 10:10 AM. ORDERED A <br /> BREAKFAST BURRITO AND FOUND A DEAD"BABY FLY' IN THE BURRITO AND ONE IN THE BAG OF RESTAURANT <br /> CHIPS. RESTAURANT WAS VERY DIRTY." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #23 Rodents, Insects or Animals Inside Facility <br /> OBSERVATIONS:There are flies in the facility (kitchen and seating area).Continue corrective action measures to <br /> reduce/eliminate fly activity. <br /> Facility was serviced by Clark Pest Control on 8-16-19 (this date) at 9:35-10:22am. Pest Away Service conducted (sprayed <br /> for spiders -Suspend Poly Zone LV and sprayed for ants/spiders- Suspend SC). <br /> Interior rodent traps were serviced and two rat captures in ceiling void area-one over the refrigerator and one by the back <br /> door area. Rats removed and traps were reset. Exterior bait stations had gnawed bait Bait replaced at exterior bait station. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats, mice), cockroaches, flies.( 114259.1, 114259.4, <br /> 114259.5) <br /> #44 Premises: Clean/Litter Free; Vermin-Proof <br /> OBSERVATIONS:There are large gaps remaining at the bottom of the rear screen door. Eliminate gaps by 1 week.
