(continued) <br />- name & job title of each employee <br />- written job description for each position (job title) listed, including requisite skill, education, <br />qualifications, and duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />Immediately prepare and implement a complete hazardous waste management training program for this <br />facility in accordance with Title 22 regulations and ensure that all employees complete this training at <br />least annually. Submit a copy of the training program and records the the EHD by May 26, 2011. <br />60, 61, & 62. Manifest numbers 002151594SKS (9/11/09) and 002184104SKS (2/15/10) were found <br />without a signed off copy from the destination facility. Manifest numbers 001773994SKS (3/30/09), <br />001991461 SKS (6/18/09), 002184061 SKS (1/6/10), and 003329932FLE (5/25/10) were not found on <br />site. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility <br />on site for three years and have them readily available for review. If the generator did not receive a copy <br />of the manifest with the handwritten signature of the owner or operator of the facility to which the <br />generator's waste was submitted within 35 days of the date the waste was accepted by the initial <br />transporter, the generator shall contact the transporter and/or the owner or operator of the designated <br />facility to determine the status of the waste. If the generator did not receive a copy of the signed off <br />copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the <br />generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a <br />cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results <br />of those efforts. Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. <br />By May 26, 2011, submit to the EHD a copy of the signed off copy of the manifests or a copy of the <br />exception report submitted to DTSC. <br />83. Used fluorescent bulbs were observed unlabeled. A generator shall label or mark universal waste <br />with the words 'Universal Waste' and the type of waste (alkaline batteries, mercury containing <br />equipment, fluorescent bulbs, electronic devices, CRTs, aerosol cans).
