W A7 <br /> (or) <br /> 2) Submit an application package for a minor waste tire facility permit <br /> (see attached application package forms) by April 23, 2005. The <br /> application package shall include the following: <br /> 1) Minor Waste Tire Facility Permit Application (CIWMB 500). <br /> 2) Operation Plan (CIWMB 501). <br /> 3) Environmental Information (CIWMB 502). <br /> 4) Emergency Response Plan (CIWMB 503). <br /> Further, please be advised that any further transportation of waste tires to this r <br /> location is a violation of state statute. Under the law, it does not matter whether <br /> the property owner/operator brought the waste tires on site, allowed others to �- <br /> bring the tires on site or the tires were brought on site without permission, such <br /> as from illegal dumping. In all cases, the property owner and/or operator are <br /> required to remove the tires. <br /> Please be advised that if the waste tires are not removed, or if the <br /> application package is not submitted, by April 23, 2005, the issue will be <br /> forwarded to the CIWMB for further enforcement which may include the <br /> issuance of a Clean-up & Abatement Order setting deadlines and penalties. <br /> To avoid this action, please advise us prior to April 23, 2005 of when the tires <br /> have been removed and furnish us with copies of all manifests used for the <br /> transport of the tires. <br /> Should you have any questions regarding this letter, please call me at (209) 953- <br /> 7868 or Robert McClellon, Supervisor REHS at 468-0332.
