W A7 (or) 2) Submit an application package for a minor waste tire facility permit (see attached application package forms) by April 23, 2005. The application package shall include the following: 1) Minor Waste Tire Facility Permit Application (CIWMB 500). 2) Operation Plan (CIWMB 501). 3) Environmental Information (CIWMB 502). 4) Emergency Response Plan (CIWMB 503). Further, please be advised that any further transportation of waste tires to this r location is a violation of state statute. Under the law, it does not matter whether the property owner/operator brought the waste tires on site, allowed others to �- bring the tires on site or the tires were brought on site without permission, such as from illegal dumping. In all cases, the property owner and/or operator are required to remove the tires. Please be advised that if the waste tires are not removed, or if the application package is not submitted, by April 23, 2005, the issue will be forwarded to the CIWMB for further enforcement which may include the issuance of a Clean-up & Abatement Order setting deadlines and penalties. To avoid this action, please advise us prior to April 23, 2005 of when the tires have been removed and furnish us with copies of all manifests used for the transport of the tires.
