Lodi Program: HW <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II, or MINOR-Notice to Comply) <br /> #61 1 did not observe the documentation for attempt to determine the status of a hazardous when <br /> manifest copy not received for manifests 000777586GBF and 000777634GBF. If after 35 days the <br /> manifest copy is not received, the generator must make an attempt to determine the status of the <br /> hazardous waste. Correct immediately. <br /> #62 1 did not observe the exception report for the manifests 000777586GBF and 000777634GBF. If <br /> after 60 days the manifest copy is not received, the generator must submit an exception report. Correct <br /> immediately. <br /> #60 and 63 1 did not observe any manifests for disposal of used oil in 2007. Manifests 000777586GBF <br /> and 000777634GBF were not available for review. Disposal records for hazardous waste must be <br /> retained for three years. Correct immediately. Submit copy of these manifests to this office by 7-1-10. <br /> #78 1 observed an open five gallon bucket of universal waste batteries. Universal waste batteries must <br /> be stored in a closed container and in manner that prevents breakage and release to the environment. <br /> Correct immediately. <br /> #79 1 observed approximately 40 universal waste lamps not stored in a closed container. Universal <br /> waste lamps must be stored in a closed container and in a manner that prevents breakage and release <br /> to the environment. Correct immediately. <br /> #83 1 observed that the universal waste lamps and universal waste batteries were not labeled as <br /> universal waste lamps and universal waste batteries. Universal waste must be labeled to identify type of <br /> waste. Correct immediately. <br /> #84 1 did not observe documentation for how long the universal waste has been accumulating. Universal <br /> waste must be disposed of within one year of the accumulation start date. Correct immediately. Submit <br /> copy of the disposal record to this office by 7-1-10. <br /> #85 1 did not observe any records for disposal of the universal waste lamps, electronics, or batteries. <br /> Records of each shipment of universal waste must be retained for three years.
