Lodi Program: HW SUMMARY OF VIOLATIONS (CLASS I,CLASS II, or MINOR-Notice to Comply) #61 1 did not observe the documentation for attempt to determine the status of a hazardous when manifest copy not received for manifests 000777586GBF and 000777634GBF. If after 35 days the manifest copy is not received, the generator must make an attempt to determine the status of the hazardous waste. Correct immediately. #62 1 did not observe the exception report for the manifests 000777586GBF and 000777634GBF. If after 60 days the manifest copy is not received, the generator must submit an exception report. Correct immediately. #60 and 63 1 did not observe any manifests for disposal of used oil in 2007. Manifests 000777586GBF and 000777634GBF were not available for review. Disposal records for hazardous waste must be retained for three years. Correct immediately. Submit copy of these manifests to this office by 7-1-10. #78 1 observed an open five gallon bucket of universal waste batteries. Universal waste batteries must be stored in a closed container and in manner that prevents breakage and release to the environment. Correct immediately. #79 1 observed approximately 40 universal waste lamps not stored in a closed container. Universal waste lamps must be stored in a closed container and in a manner that prevents breakage and release to the environment. Correct immediately. #83 1 observed that the universal waste lamps and universal waste batteries were not labeled as universal waste lamps and universal waste batteries. Universal waste must be labeled to identify type of waste. Correct immediately. #84 1 did not observe documentation for how long the universal waste has been accumulating. Universal waste must be disposed of within one year of the accumulation start date. Correct immediately. Submit copy of the disposal record to this office by 7-1-10. #85 1 did not observe any records for disposal of the universal waste lamps, electronics, or batteries. Records of each shipment of universal waste must be retained for three years. Correct immediately. Submit copies of these records to this office by 7-1-10. Complete and Submit the Return to Compliance Certification, corrective action statements for each violation, and any supporting documentation to this office by 7-1-10. Note: 1.4 tons of HW generated in 2009. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($'I'TS).
