Attachment P <br /> Monitoring Program <br /> Application Requirements <br /> Discuss the planned monitoring program. This should be thorough,including maps <br /> showing the number and location of monitoring wells as appropriate and discussion of <br /> monitoring devices,sampling frequency, and parameters measured. If a manifold <br /> monitoring program is utilized,pursuant to §146.23(b)(5), describe the program and <br /> compare it to individual well monitoring. <br /> The following program of well data acquisition and review is proposed for purposes of <br /> prudent well operation, early recognition of any developing problems, and compliance with <br /> regulatory agency requirements. These monitoring activities are to include the use of <br /> continuously recording devices to monitor the mechanical integrity of the well, and to <br /> record the physical conditions occurring at the well head during injection (temperature, <br /> pressure,flow rate). In addition,coordinated programs of inspections, samplings and <br /> analyses,tests,loggings, and reviews are described below,together with a schedule of the <br /> reports to be submitted. <br /> Approach <br /> Each of the various monitoring requirements applies equally to each injection well at the <br /> STIG-LEC facility. Therefore,the information in this attachment will be presented by <br /> monitoring type and not by individual well. <br /> Mechanical Integrity Monitoring/Reporting Program <br /> Continuously Recording Monitor Devices <br /> The mechanical integrity of the well will be monitored by the use of instrumentation devices <br /> that continuously record the annular pressure between the 4-1/2-inch tubing and the 7-inch <br /> casing above the packer. The annular space will be kept full with water and pressurized to a <br /> minimum of 100 psig with a pressurized receiver located at the wellhead. During each work <br /> shift,the water level in the receiver will be recorded by the operator,and the monitoring <br /> devices will be inspected and checked to confirm that they continue to operate properly. <br /> Daily and monthly minimum,average, and maximum values for these two parameters will <br /> be compiled monthly and reported quarterly. <br /> Annual Mechanical Inspection and Evaluation <br /> Each year, a program of mechanical inspection and evaluation will be performed.
