. 1 <br /> 3.2 Well Destruction Activities <br /> Task 4: Well Abandonment. All field procedures will be overseen by Muir or under the <br /> supervision of a California Professional Geologist.The RWQCB and the SJCEHD will be <br /> property informed prior to the implementation of any abandonment activities proposed in the <br /> work plan. <br /> Soil Gas Well and Soil Gas Sampling Point Destructions: The seventeen shallow soil gas <br /> wells and sample points depicted on Figure 7 will have traffic boxes,cement and sand removed <br /> from the subsurface by jackhammer. After all well construction material is removed from the <br /> subsurface, each soil gas well or sample point will have all well or sample point soil and <br /> construction material removed by hand augur and/or jack hammer to total depth as shown in <br /> Table 1. The depth of each boring will be measured and recorded. <br /> A grout mix description will be obtained from SJCEHD and will be described in detail within the <br /> well abandonment permit applications prepared by the C57 water well contractor. The C57 <br /> water well contractor will conduct the grout placement within each boring under the oversight of <br /> the SJCEHD onsite inspectors. All debris will be placed in a 55-gallon USDOT drums pending <br /> determination for disposal. <br /> Groundwater Monitoring Well Destructions: The three groundwater monitoring wells(LA- <br /> MW 1,MW2 and MW4 will have traffic boxes removed. The explosives contractor will <br /> evaluate each wellbore log and other information in order to determine the preferential location <br /> and size of the explosive charges to place in each wellbore. After the charge has been placed in <br /> the monitoring well by the blaster the C57 well contractor will fill the well casing using a tremie <br /> pipe with the grout mixture described above. After the well bore has been filled with grout the <br /> explosives contractor will initiate the shot in accordance with the Blasting Safety Plan and the <br /> explosives operation permit. The wellbore will be filled with additional grout until the onsite <br /> SJCEHD inspector is satisfied that no additional grout mix is necessary. <br /> All investigative derived material will be removed from the site and placed in 55-gallon USDOT <br /> drums pending disposal evaluation. <br /> 3.3 Reporting <br /> Task 5: Reporting: A California Department of Water Resources(DWR)Well Completion <br /> Report(WCR)Form 188-electronic for each of the monitoring wells will be prepared by the C57 <br /> drilling contractor and filed with the DWR. Copies of the Form 188 logs will be provided as <br /> part of a Report of Well Destruction that will be prepared by the Consultant and submitted to <br /> the RWQCB and SJCEHD for review and approval. The Report of Well Destruction will be <br /> uploaded onto the SWRCB Geotracker website. <br /> It is anticipated that after all investigative derived waste has been property disposed of and the <br /> RWQCB and SJCEHD have reviewed and approved of the Report of Well Destruction that the <br /> Leaking Underground Storage Tank Case on the site will be formally closed. <br /> Lodi Airport Well Destruction Work Plan\10272020.wpd 5 <br />
