i <br /> 3.0 WELL DESTRUCTION SCOPE OF WORK <br /> 3.1 Health and Safety,Permitting and Utility Clearance <br /> Task 1: Health and Safety Plan. A Site Health and Safety Plan(HASP)has been prepared in <br /> accordance with Occupational Safety and Health Guidance Manual for Hazardous Waste Site <br /> Activities(National Institute for Occupational Safety and Health Administration) and federal(29 <br /> CFR 1910.120) and state(Title 8, Cal. Code Reg., Section 5192)regulations(Appendix 3). <br /> All field personnel involved with this scope of work will be trained in hazardous materials <br /> communication and will have completed a 24-or 40-hour Health and Safety Course as specified <br /> in OSHA Hazardous Waste Operations and Emergency Response Standard [29 CFR 1910.120 <br /> (c)],and 8-hour annual refresher courses as necessary. <br /> Task 2 : Permitting.Permits will be obtained from the San Joaquin County Environmental <br /> Health Department(SJCEHD) for destruction of the three groundwater monitoring wells and <br /> seventeen shallow soil gas and/or soil sampling points. The three groundwater monitoring wells <br /> will be destroyed by explosive destruction method. A specialty well abandonment company that <br /> possesses the appropriate explosive permits and licenses will be used to conduct the explosive <br /> operations. It is anticipated that the explosive blaster contractor will be Tylor McMillan's Well <br /> Service,LLC (McMillan's) A permit from the San Joaquin County Sheriffs office will be <br /> obtained by the subcontractor drilling company. Appendix 4 provides individual workplans for <br /> each of the three groundwater monitoring wells LA-MW 1,2 and 4 by explosive destruction. <br /> Also presented in Appendix 4 is the Blasting Safety Plan. <br /> Prior to mobilization, each well or sampling point will be clearly marked and a utility clearance <br /> will be obtained through Underground Service Alert. <br /> Task 3: Utility Clearance Survey. Consultant will conduct both electromagnetic and magnetic <br /> surveys of the former UST system area on the Golden Aviation site to determine the location of <br /> underground facilities at the site such as buried electrical,power,and sewer lines. Underground <br /> Service Alert of Northern California will be contacted to clear the drill sites after they are <br /> marked, but at least 72 hours prior to commencement of field work. <br /> Lodi Airport Well Destruction Work Plan\10272020.wpd 4 <br />
