e. A video tape of road conditions on Waverly Road from Shelton to Flood Road is required <br /> prior to the start of excavation. The condition of Waverly Road shall be reviewed with the <br /> Department of Public Works prior to the start of excavation and every five years <br /> thereafter. If it is determined necessary by the Department of Public Works during the <br /> excavation, the applicant shall construct an acceleration land and/ or a left-turn lane on <br /> Waverly Road to permit safe egress and ingress from the site. Road construction and <br /> design shall be subject to approval by the Department of Public Works. The applicant <br /> shall pay fees for the plan-check, inspection, and construction cost. <br /> f. A bond or cash deposit in the amount acceptable to the Department of Public Works shall <br /> be filed with the Department with a guarantee that if the permittee fails, refuses, or <br /> neglects to repair, under permit, any damages to Waverly Road which may result from <br /> the permittee's operations, the Department of Public Works may do to the work <br /> necessary to restore Waverly Road Equal to, or better than, its original condition by using <br /> the funds from said bond or cash deposit. <br /> g. A bond or cash deposit in the amount acceptable to the Department of Public Works shall <br /> be filed with the Department with a guarantee that if the permittee fails, refuses, or <br /> neglects to reclaim the project site, the Department of Public Works may do the work of <br /> reclamation by using the funds from the said bond or cash deposit. The progress of <br /> -reclamation shall be reviewed with the Department of Public Works every five years and <br /> at the end of excavation. <br /> h. The Traffic Impact Mitigation Fee will be required for this development. The fee is due <br /> and payable at the time of building permit application. The fee is $200.51 per daily trip- <br /> end in the Linden area. The fee shall be automatically adjusted annually by the <br /> Engineering Construction cost Index as published by the Engineering News Record. <br /> 3. PUBLIC HEALTH SERVICES: <br /> Ordinance Reguiremen"ts: <br /> The Ordinance Requirements listed below have been identified as pertinent to this project. <br /> Other Ordinance Requirements may also apply. Ordinance Requirements cannot be modified. <br /> a. No excavation is permitted into the ground water table without specific approval from the <br /> Regional Water Quality Control Board, Central Valley Region. <br /> b. The materials form excavation shall not be deposited into the water course nor in any <br /> way contribute to the pollution thereof. <br /> C. Provide an approved potable water supply for all employees. <br /> d. Provide approved sanitary facilities for all employees. <br /> e. Open pump, and backfill the septic tank, seepage pit or excavation under permit and <br /> inspection by the Environmental Health Division when the existing residences are <br /> vacated or removed. <br /> f.
