In Work Plan, Ramage provides a proposed scope of <br /> work for destruction of 8 monitoring wells installed at the subject site as follows: <br /> • Over-drill monitoring wells MW-1 through MW-6 using 8-inch diameter augers through <br /> each well's total depth to remove all well contents; <br /> • Utilize a tremie pipe extending to within 5 feet of each well's total depth to install a cement <br /> seal to within 3 to 5 feet below surface grade; and <br /> • Pressure-grout monitoring wells MW-7 and MW-8, including the installation of a mushroom <br /> grout cap on both wells; <br /> The EHD approves of the scope of work presented in Work Plan with the following conditions: <br /> • Mix neat portland cement at a ratio of 5 to 6 gallons of clean, potable water per 94 <br /> pounds of grout; <br /> • All investigation-derived wastes (IDW), including soil cuttings, debris, and groundwater <br /> generated from soil borings and well installations, shall be containerized for proper <br /> disposal and the containers labeled to characterize the waste: hazards, composition, <br /> generation site address, generator's name, and date of accumulation; and <br />
