Stockton Metropolitan Airport Page 2 of 2 <br /> Former Fuel Station June 30, 2015 <br /> 5000 South Airport Boulevard <br /> Stockton, California 95206 <br /> • A well destruction report describing well destruction field activities and including the <br /> receiving facility-signed manifests for the disposed IDW must be uploaded to <br /> Geotracker. <br /> To proceed with well destruction permit issuance the following must be submitted to the EHD: <br /> • Two EHD Well and Boring Permit Application forms (one for each parcel); <br /> • Master File Record form (MFR); <br /> • Licensed Contractor's Declaration (LCD); and <br /> • A fee of $910 (5 x $130 + 2 x $130). Additional time spent by EHD staff on activities <br /> related to the permitted work beyond the initial pre-paid fee will be billed at the current <br /> hourly rate of$130. <br /> A 60-day public comment period regarding the pending case closure must be completed prior to <br /> issuance of well destruction permits. After the permits have been issued, the EHD requires that <br /> inspections be scheduled at least 5 working days prior to the commencement of field work. <br /> This EHD approval is limited to only the well destruction methods proposed in the work plan. <br /> The approval is not a concurrence or endorsement for any other conclusions, conditions or <br /> implications contained in the Ramage Work Plan, Well Destructions and Site Restoration (Work <br /> Plan) dated June 19, 2015. <br /> As directed by EHD letter dated April 16, 2015, on June 16, 2015, Ramage attempted to collect <br /> groundwater samples frorn a water supply well located approximately 350 feet west of the <br /> subject site (Stockton Field Annex Well No. 1) and a water supply well located approximately <br /> 550 feet southwest of the subject site (County Well #2). Rarnage was unable to collect a sample <br /> from Stockton Field Annex Well No. 1. County Well #2 was not located.
