Stockton Metropolitan Airport Page 2 of 2
Former Fuel Station June 30, 2015
5000 South Airport Boulevard
Stockton, California 95206
• A well destruction report describing well destruction field activities and including the
receiving facility-signed manifests for the disposed IDW must be uploaded to
Geotracker.
To proceed with well destruction permit issuance the following must be submitted to the EHD:
• Two EHD Well and Boring Permit Application forms (one for each parcel);
• Master File Record form (MFR);
• Licensed Contractor's Declaration (LCD); and
• A fee of $910 (5 x $130 + 2 x $130). Additional time spent by EHD staff on activities
related to the permitted work beyond the initial pre-paid fee will be billed at the current
hourly rate of$130.
A 60-day public comment period regarding the pending case closure must be completed prior to
issuance of well destruction permits. After the permits have been issued, the EHD requires that
inspections be scheduled at least 5 working days prior to the commencement of field work.
This EHD approval is limited to only the well destruction methods proposed in the work plan.
The approval is not a concurrence or endorsement for any other conclusions, conditions or
implications contained in the Ramage Work Plan, Well Destructions and Site Restoration (Work
Plan) dated June 19, 2015.
As directed by EHD letter dated April 16, 2015, on June 16, 2015, Ramage attempted to collect
groundwater samples frorn a water supply well located approximately 350 feet west of the
subject site (Stockton Field Annex Well No. 1) and a water supply well located approximately
550 feet southwest of the subject site (County Well #2). Rarnage was unable to collect a sample
from Stockton Field Annex Well No. 1. County Well #2 was not located. Further directive
regarding the sampling and destruction of inactive water supply wells is forthcoming.
If you have any questions please contact Nuel Henderson by email at nhenderson@sjcelid.com
or by phone at (209) 468-3436.
S".141
Gabriel Gonzalez, REHS Nuel C.
