Provide a complete closure plan by 11/19/04. <br /> A revised Closure Plan with current quotation for Transportation, disposal and service for <br /> closure of clarifier, filter press,tanks,and associated piping/conduit is enclosed with this <br /> report as: Attachment(4) <br /> Note: <br /> "This facility does not have the financial assessment bond filled out to the correct agency. <br /> Provide documentation supporting the transfer of the bond from the DTSC to San Joaquin <br /> County Environmental Health Department by 11/19/04. <br /> Sumiden Wire has made numerous efforts to comply with this item. Including many <br /> telephone calls to various people and places. Most recent has been to Steve at DTSC in <br /> Sacramento who has thus far been unsuccessful in locating the original financial bond in the <br /> amount of$50,000.00 issued to DTSC on December 29, 1994. My last voice message to <br /> Steve at DTSC(916-255-3751)on November 16, 2004 was to advise him to look into the <br /> past records of Sumiden Wire Products Corp, and find the December 1994/January 1995 <br /> copy of"TIERED PERMITTING PHASE I ENVIRONMENTAL ASSESSMENT <br /> CHECKLIST"and to check if the bond might be attached to it. Attachment(7)is a copy of <br /> receipts for Certified Mail for both Dept. of Toxic Substance Control and San Joaquin <br /> County Environmental Health Div. That they both received copies in which the ORIGINAL <br /> bond was sent with the report package to DTSC. In addition,Attachment (5)and Attachment <br /> (6)are copies of faxes sent in an effort to make the name change on the bond.
