Since this vapor recovery was not <br /> observed to have secondary containment, this should be changed to "none". <br /> 91 tank-The vapor recovery secondary containment is listed as"fiberglass". Since this vapor recovery was not <br /> observed to have secondary containment, this should be changed to "none". The vent secondary containment is not <br /> marked and should be marked as"none". <br /> diesel-The riser pipe primary containment is not marked and should be marked"steel'. <br /> Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, <br /> update the required information, and submit for review by the EHD. <br /> This is a repeat violation, Class II. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted. The plot plan submitted did not contain the legend and did <br /> not include the locations of the annular sensors. A site plan must be submitted identifying the locations where <br /> monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a Class II violation. <br /> 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. <br /> A monitoring system certification, leak detector testing, and spill container testing were performed on September 22, <br /> 2014 and a test report has not been submitted to the California Environmental Reporting System (CERS). A copy of <br /> the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. <br /> This is a Class II violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. Alarm logs are not being <br /> maintained on site. The only maintenance record on site was dated June 22, 2015 for an alarm on June 21, 2015. <br /> The alarm history indicated alarms on 7/21/15, 6/21/15, 12/22/14, 12/4/14. EHD records indicate fieldwork in February <br /> and March 2015 including replacement of the 87 leak detector, the dispenser 1/2 diesel impact valve, and the 87 <br /> turbine relay. These records shall be maintained on site for at least three years. Monitoring records include: (1)date <br /> and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of <br /> any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges <br /> or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br /> reconciliations.
