A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a Class II violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the designated operator <br /> monthly inspection reports and failed to check that they were responded to appropriately. The missing alarms <br /> include: L3 (annular)6/21/15 fuel and sensor out alarms, L3 (annular) 7/21/15 fuel alarm, T1 (87) 12/4/14 probe out <br /> alarm, and T1 (87) 12/22/14 probe out alarm. The attached alarm histories do not include tank alarms (T1, T2, T3)for <br /> the following months: October and December 2014 and January, February,April, May, July,August and September <br /> 2015. The July 21, 2015 alarm was documented on the August 2015 monthly report; however, the designated operator <br /> did not check that the alarm was responded to appropriately. During the monthly inspection, the designated operator <br /> shall review the alarm history for the previous month, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br /> monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a repeat violation, Class ll. <br /> 315 HSC 25290.2(c)(1) Primary containment not tight(Jul 2003-Jun2004). <br /> The diesel filter at dispenser 1,2 was observed to be leaking into the UDC and approximately one-half cup of diesel <br /> was observed in the UDC, indicating a leak in the primary piping. All primary containment for the UST system must <br /> be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component. <br /> This is a Class II violation. <br /> 320 HSC 25290.1(c3), 25290.2(c3) Secondary containment not constructed to prevent water intrusion. <br /> Water was found in the diesel STP sump. Secondary containment shall be constructed to prevent any water intrusion <br /> into the system by precipitation, infiltration, or surface runoff. Immediately remove this liquid, make a hazardous <br /> waste determination per Title 22 hazardous waste regulations, and manage accordingly. Immediately contact a <br /> properly licensed, trained, and certified contractor to address the water intrusion into the diesel STP sump under <br /> permit and inspection of the EHD. <br /> This is a Class II violation. <br /> 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting <br /> a 3-gallon per hour leak at 10 psi.
