<br />SUMMARY OF VIOLATIONS
<br />CLASS I, CLASS II, or MINOR -Notice to Comply)
<br />This is a Routine and Monitoring System Certification inspection. A checklist only was provided to the
<br />facility on the day of the inspection (September 23) due to unexpected equipment malfunction. This
<br />report supersedes all previous versions. The following are Class II violations observed on site: #201,
<br />204, 205, 206, 301/303, 305, 999.
<br />201. No visual observation records or daily alarm logs during the last twelve months when alarm history
<br />from the monitoring panel could not be printed for monthly designated operator. These records shall be
<br />kept on site for at least 3 years. Immediately locate and maintain the alarm logs on site, and submit a
<br />copy to EHD by October 26, 2013.
<br />204. Failed to perform monthly inspections by the designated operator in May and June 2013. (Operator
<br />had informed EHD the monthly designated operator inspection in May and June 2013 were not
<br />conducted because the previous designator had suddenly retired, and had since retained Afford -Test as
<br />the new designated operator in July 2013. Nevertheless, this is still a violation). During the monthly
<br />inspection, the designated operator shall verify that all facility employees have been trained. Ensure that
<br />designated operators performing monthly inspections at this facility are including all of the required
<br />information on the reports.
<br />204. The designated operator failed to verify and document all alarms on the designated operator
<br />monthly inspection reports from September 2012 to April 2013, and failed to check if any alarm was
<br />responded to appropriately. The attached alarm history on the referenced reports included only a single
<br />heading for the L1 sensor (but with no alarm history for the L1 either) and no other sensors, which the
<br />designated operator interpreted and commented on the reports as no alarm for the month. During the
<br />monthly inspection, the designated operator shall review the alarm history for the previous month, check
<br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history
<br />with documentation taken in response to any alarms to the monthly report. Immediately review your
<br />records and logs, and submit a copy of any alarm found and the response you have taken for them to
<br />EHD by October 26, 2013, and ensure that designated operators performing monthly inspections at this
<br />facility are including all of the required information on the reports.
<br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
