Fremont, Stockton <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS II, or MINOR -Notice to Comply) <br />205. The July and August 2013 designated operator monthly inspection reports were not found on site. <br />Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br />Locate and ensure that copies of the previous twelve months of designated operator monthly inspection <br />reports are maintained on site. Submit copies of the July and August 2013 designated operator <br />inspection report to the EHD by October 26, 2013. <br />206. Current training log indicated only one employee, Alam Zafar, had been trained by the new <br />designated operator; and the previous training log by the last designated operator Mark Fairbanks could <br />not be located to verify if other employees had been trained. The designated operator shall train facility <br />employees for which he or she is responsible in the proper operation and maintenance of the UST <br />system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by October 26, 2013. <br />301 & 303. The complete monthly alarm history could not be printed, which caused it to be 1) not <br />included in the designated operator monthly inspection reports (see above Item #204) , 2) unable to be <br />printed before the monitoring system certification observed today, 3) and after the certification, to print <br />only the alarms set off today (but does not include the last three alarms which the Veeder Root <br />Monitoring System is known for). The underground storage tank system shall be operated to prevent <br />unauthorized releases and to ensure continuous monitoring of the tank system as required by CCR 2630 <br />(d). Correct immediately by having a properly licensed, trained, and certified contractor replace the <br />failed component with a functional component (LG 113-x listed, if applicable) under permit. If the failed <br />component can't be replaced immediately, there is a possibility that the UST system may be red tagged <br />to prevent fuel inputs. Submit permit application by October 26, 2013. <br />303. The diesel STP sump sensor (1-6), Diesel Dispenser #1/2 UDC sensor (1-8), and sensor out shutting <br />off of the diesel STP were removed from the monitoring system programming, and therefore could not <br />detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a <br />leak at the earliest possible opportunity. Technician reprogrammed the monitoring panel, retested the <br />sensors and sensor out, and they passed.
