Fremont St., Stockton, CA 95205 (SR0057332) <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the permit application to install ISD, and a Veeder Root carbon canister vapor polisher (CCVP) <br />system at the above -referenced facility. No concrete will be cut for electrical conduit. Bollards <br />will be installed in front of the vent risers where the CCVP is to be installed as drawn on the <br />blueprint. The permit has been approved with the following conditions. <br />Conditions of approval: <br />1. Contact the local fire department, local planning or building department and the San <br />Joaquin Valley Air Pollution Control District for any applicable permits. <br />2. Any changes shall be submitted to the San Joaquin County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda supercede <br />the previous ones. <br />3. Before the alarm history is cleared by the "cold start" of the ISD installation, a <br />current printout must be created and available for EHD inspection. Inspection may <br />occur during the monitoring system functionality test. <br />4. If concrete is broken near the existing underground storage tanks (UST), tank secondary <br />containment test must be performed on the UST in accordance to manufacturer's <br />guidelines. Testing must be done after returning the facility to operating condition. <br />Results must be submitted to EHD within 7 days of returning the facility to operating <br />condition. <br />5. All components must be certified by an independent testing organization and labeled as <br />required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See California Code of <br />Regulations (CCR), Title 23 Chapter 16 Table B for complete list). The component must <br />bear the appropriate mark as required by the independent testing organization. <br />6. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a.
