SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.siaov.oro/ehd <br /> CONTINUATION FORM Page: 5 of 6 <br /> OFFICIIAL INSPECTION REPORT Date: 08/19/13 <br /> Facility Address: Prolube Oil Change Centers 808 S Cherokee Ln Lodi CA 95240 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I CLASS II,or MINOR-Notice to Comply) <br /> months after the date of their employment or assignment to a facility, or to a new position at a facility. <br /> Employees hired after the effective date of these regulations shall not work in unsupervised positions <br /> until they have completed the training requirements of subsection (a) of this section. (c) Facility <br /> personnel shall take part in an annual review of the initial training required in subsection (a) of this <br /> section. (d) The owner or operator shall maintain the following documents and records at the facility: (1) <br /> the job title for each position at the facility related to hazardous waste management, and the name of the <br /> employee filling each job; (2) a written job description for each position listed under subsection (d)(1) of <br /> this section. This description may be consistent in its degree of specificity with descriptions for similar <br /> positions in the same company location or bargaining unit, but shall include the requisite skill, education, <br /> or other qualifications, and duties of facility personnel assigned to each position; (3) a written description <br /> of the type and amount of both introductory and continuing training that will be given to each person <br /> filling a position listed under subsection (d)(1) of this section; (4) records that document that the training <br /> or job experience required under subsections (a), (b), (c), of this section has been given to, and <br /> completed by, facility personnel. (e) Training records on current personnel shall be kept until closure of <br /> the facility. Training records on former employees shall be kept for at least three years from the date the <br /> employee last worked at the facility. Personnel training records may accompany personnel transferred <br /> within the same company. Correct immediately by keeping a copy of the training records at this facility <br /> and submitting a copy to the EHD by September 19, 2013. <br /> 125. Copies of used oil hazardous waste disposal records for January 2012 through July 2012 were not <br /> available for review upon request. Hazardous waste generators shall retain copies of all manifests <br /> signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site <br /> for three years and have them readily available for review. Immediately locate a copy of all manifests <br /> and receipts for the last three years, maintain them on site, and submit copies to the used oil disposal <br /> receipts to the EHD by September 19, 2013. <br /> 133. This facility does not have a hazardous waste tank assessment certified by a professional engineer <br /> for the used oil tank. At this time the facility has a preliminary written assessment in which the <br /> professional engineer has noted discrepancies on page 2 and A-3 that the professional engineer has <br /> identified as needing to be corrected before he will provide final certification. Owners or operators of tank <br /> systems or components shall ensure that the foundation, structural support, seams, connections, and <br /> pressure controls are adequately designed and that the tank system has sufficient structural strength, <br /> compatibility with the waste(s) to be transferred, stored or treated, and corrosion protection so that it will <br /> not collapse, rupture, or fail. The owner or operator shall obtain a written assessment reviewed and <br /> certified by an independent, qualified, professional engineer, registered in California in accordance with <br /> ALL EHD STAFF TIME ASSOCIATED WITH FARING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector.
