This assessment shall be obtained prior to <br /> placing the tank system in service, and shall be kept on file at the facility. Correct immediately by <br /> obtaining the required tank assessment for the used oil tank per Title 22 hazardous waste regulations. <br /> Submit a copy of the professional engineer certified hazardous waste tank assessment to the EHD. <br /> 135. A routine inspection was conducted on August 16, 2012. The return to compliance statements <br /> submitted to the EHD for this inspection are incomplete. The facility must submit a written response <br /> within thirty days of receiving an inspection report. Correct immediately by completing the return to <br /> compliance statements for the August 16, 2012, inspection report and submit them to the EHD by <br /> September 19, 2013. <br /> Complete and submit the return to compliance certification form, corrective action statements for each <br /> violation, and any supporting documentation to the EHD by September 19, 2013. <br /> Ensure that all required information has been entered into the online California Electronic Reporting <br /> System. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHO'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Aris Cacapit (209) 468-3453 <br /> EHD 23-02-003 Rev 04119/12 CONTINUATION FORM <br />
