A hazardous waste generator shall not treat, <br /> store, dispose of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to <br /> the DTSC web site http://www.dtse.ca.gov/[DManifest/PERMHWID.cfm for the form to reactivate the ID number . <br /> Immediately reactivate your State ID number and submit evidence to the EHD. <br /> This is a minor violation. <br /> 108 CCR 66265.53 Failed to maintain a hazardous waste contingency plan onsite and submit to required agencies. <br /> The contingency plan was not available at the time of inspection. Andrew Roa, Manager could not provide a copy of <br /> the facility contingency plan to review during the inspection. A copy of the contingency plan and all revisions to the <br /> plan shall be maintained at the facility at all times. Immediately locate a copy of the contingency plan for the facility <br /> and submit a copy of the plan to the EHD. <br /> This is a repeat violation, Class II. <br /> 111 CCR 66265.16(d) Failed to maintain complete personnel training records. <br /> Employee training records for the management of hazardous waste could not be found during the inspection. Andrew <br /> Roa, Manager confirmed that his employees have been trained on hazardous waste management during the monthly <br /> safety meetings. Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to each <br /> person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former employees <br /> shall be kept for at least three years from the date the employee last worked at the facility. Ensure that employee <br /> training is properly documented. <br /> This is a repeat violation, Class II. <br /> 123 HSC 25404(e)(4) Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS) for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> This is a Class II violation. <br /> Page 4 of 6 <br />
