BALDWIN <br />COORDINATOR <br />Following is supplemental statements to the inspections conducted at Eddie's Pizza facilities: <br />Eddie's Pizza <br />1419 S. El Dorado Street\ <br />December 16, 1998 <br />Robert Lopez and myself conducted a verification inspection with the storage manager, <br />Ms. Zuniga <br />2. Ms. Zuniga was asked about the bulk Carbon Dioxide tank and she stated it had been <br />removed the day before. <br />3. One each 501b CO2 cylinders was inside the storage on line. <br />4. Behind the facility is a separate building which had three each 501b CO2 cylinders with two <br />full and one empty. Also one each 201b cylinder full. The cylinders were not chained as <br />required by fire code. <br />5. None of the cylinders were identified as full or empty. <br />Eddie's Pizza <br />1048 E. Waterloo Rd <br />December 16, 1998 <br />1. Robert Lopez and myself conducted a verification inspection with the owner George <br />Badway and the storage manager. <br />2. Located in a room, that is only accessible from the outside, there was five each full 501b <br />CO2 cylinders, one each empty 501b CO2 cylinder, and one each empty 201b CO2 cylinder. <br />The cylinders were not properly chained as required by fire code. <br />3. Mr. Badway demanded that we verify the amounts in the cylinders to prove rather he was <br />above the threshold quantity. I explained to Mr. Badway due to the various chemicals that <br />OES regulates, it was impossible to have a measuring device for everything that is regulated. <br />I informed Mr. Badway we use experience and best judgement to determine amounts.
