As of January <br /> 1, 2013, all businesses were required to submit all business plans electronically. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. <br /> The Business Owner/Operator Identification form was not submitted annually as part of the business plan. The last <br /> business plan submittal including the he Business Owner/Operator Identification form was submitted on <br /> 11/17/2016. The business plan shall be submitted annually This information must be current and submitted <br /> annually as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> 55 gallons of compressors used oil were found on site that have not been reported. The Hazardous Materials <br /> inventory was not submitted annually. The last Hazardous Materials Inventory submittal was on 11/17/2016.Any <br /> material that meets or exceeds the reportable quantity shall be reported in the facility's business plan.The <br /> hazardous materials inventory shall be submitted annually to CERS Immediately log into the California <br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br /> inventory, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed with all the required elements.The site map did not not address the location of <br /> Hazardous Materials Loading Areas, Internal Roads,Adjacent Streets, Storm and Sewer Drains, Evacuation <br /> Staging Area, Hazardous Materials and Storage Areas, Emergency Response Equipment, and Emergency Shutoffs. <br /> Also,the site map was not submitted annually.The last site map submittal was on 11/17/2016. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA".A site map shall be submitted as part of the business plan annually.
