Called SJGH and they said The PE 2546 BAKER <br /> 42 min Wine Group reported a small ammonia leak. They are reporting it only and do not SC 907 <br /> need assistance. It was reported by Kevin Jones (209-481-5950). The address is <br /> 17000 E HWY 120. <br /> Called Kevin Jones and left a message. Called him again and he said that leak <br /> was under a pound in size. Leak was caused by a valve packing. The seal needs <br /> to be replaced. The chiller is shut down and the leak will be repaired within 10 <br /> minutes. No injuries and no evacuations. <br /> Called Muni Naidu (Program Coordinator) and let him know about the complaint. <br /> He said to call back and confirm that the leak was fixed and I don't need to go out. <br /> Called Kevin Jones twice and left a message. Looked on CERS for alternative <br /> contacts. Called Kyle Schmidt(209-599-0451)with no answer and Chuck Mitten <br /> (Plant Manager) at 209-345-7258 with no answer. Received a message from <br /> Kevin Jones stating that the leak has been fixed. <br /> 8:48am-9:30am <br /> 08/19/2019 At 8:10 am today, Lydia Baker with SJC EHD discussed with me this complaint. I PE 2546 SAEED <br /> 180 min obtained a copy of the spill report from Lydia. At 8:26am today, I called the SC 544 <br /> reporting party, Kevin Jones with Franzia Winery, and left a voicemail to call me <br /> back. At 8:30am Kevin called me back, and told me that during the walk around <br /> yesterday at 8:30am, slight ammonia odor was discovered at Chiller#5, which is <br /> located outside. The ammonia odor was coming from the expansion valve. The <br /> amount of ammonia estimated to be less than one pound. The release was <br /> stopped immediately and the system is running again. Incident investigation has <br /> already started. I told Kevin I will be at the site today at 10:00am to inspect the <br /> point of the leak. <br /> I left the office at 9:30am and arrived onsite at 10:00am. I met with Kevin Jones <br /> and Mark McPherson. I inspected where the leak occurred and took photos. <br /> Kevin estimated the release to be less than a pound. The release occurred due to <br /> a loose expansion valve. The operators tightened the loose valve and issue was <br /> resolved and the facility continued to operate on the same day. Incident <br /> investigation in progress, Mark McPherson mentioned that they replace the valve <br /> packing annually. According to Mark and Kevin the have not had issues with the <br /> other valves on the other chillers, and the reason for the leak could be thermal <br /> expansion. I asked him if it has been replaced this year, and told me he needs to <br /> look at the records. I left the site at 10:25am. I issued a notice to abate with <br /> corrective actions to Keven Jones to send the documents showing that the valve <br /> package was replaced this year, and to email me a copy of the incident <br /> investigation report by 09/18/2019. <br /> 1 filled out the complaint info in ECR.
