Henshaw Associates, Inc. wrote the revised HASP <br /> dated March 3, 2000 and the supplemental text dated June 18, 2002. The documents <br /> were received by IHFSS on June 25, 2002. <br /> SPECIFIC COMMENTS <br /> 1. Section 1.0, Introduction. It is stated that the City of Lodi Area of Contamination is <br /> shown on Figure 1-1. Figure 1-1 is simply a map of Lodi. Please indicate the <br /> approximate size and location of the site on this map. The requested information not <br /> found in the November 8, 1999 HASP is still unavailable. Please include it in the final <br /> HASP. <br /> 2. Section 1.1, Site Description. In the plan it states, "A Site location map is shown on <br /> Figure 1"; please provide Figure 1. <br /> 3. Section 2.0, Project Safety Authority. Identify key personnel by name and specific <br /> assignment for the project. Also, include the site and office telephone numbers of <br /> key personnel and contractor/responsible party and agency personnel. <br /> 4. Section 3.2.4, Noise Exposure. Include monitoring methods and calibration <br /> information for equipment used to measure noise levels in the appendix or in the <br /> body of the HASP. <br /> 5. Section 3.2.13, Illumination Hazards. Please explain why a minimum illumination of <br /> 2 foot-candles will be maintained for sampling activities. Indicate if this is in <br /> compliance with Title 8 and other regulations. <br /> 6. Section 3.2.14, Drilling, Soil Gas Sampling and Groundwater Monitoring Hazards. <br /> Provide information on confined space entry procedures in compliance with Title 8 <br /> and other appropriate regulations. Identify all confined space entry locations and <br /> their inherent risks and specific safety precautions. <br />
