Condition No. 16 : <br /> No explosives shall be used or stored on the site. <br /> Condition No. 17 : <br /> If it is determined necessary by the Department of Public <br /> Works, the applicant shall construct an acceleration lane <br /> and/or a left-turn lane on Waverly Road to permit safe egress <br /> from the site and ingress from the public roadway. Road <br /> construction and design shall be subject to approval by the <br /> Department of Public Works. The applicant shall pay for the <br /> cost of road construction. <br /> Condition No . 18 : <br /> The applicant shall retain a certified civil engineer to con- <br /> duct annual inspections and report to the Planning Division. <br /> Such reports are to attest to the applicant ' s compliance with <br /> activities authorized or required under this permit . <br /> Condition No. 19 : <br /> Use of the proposed truck scale shall be limited to customers <br /> in the process of hauling materials from the site. <br /> Condition No. 20: <br /> Site access to Waverly Road shall be located approximately 150 <br /> feet southerly of the existing entrance and shall be <br /> constructed with a 50-foot minimum radius and a 100-foot acce- <br /> leration and deceleration lane, also due to approval of the <br /> Department of Public Works. The use of other routes for access <br /> to the site is prohibited. <br /> Condition No. 21 : <br /> Prior to the commencement of excavation, the applicant shall <br /> submit for the approval by the decision-making body: <br /> a. A revised site plan that shows site access, location of <br /> structures, excavation phasing, project boundaries, paved <br /> surfaces, and elevations of excavation depths and final sur- <br /> faces. <br /> b. A revised excavation and rehabilitation schedule which <br /> details the locations, acreages, and cubic yardages to be <br /> excavated and rehabilitated, and the timetables and sequen- <br /> ces in which these operations shall be accomplished. <br /> c. A revised rehabilitation plan which incorporates the <br /> reseeding recommendations of the Soil Conservation Service <br /> or the Agricultural Extension Service. <br /> Condition No, 22 Local Health District) : <br /> Revised Conditions - 3 - (EP-81-9) <br />
