Condition No. 16 : No explosives shall be used or stored on the site. Condition No. 17 : If it is determined necessary by the Department of Public Works, the applicant shall construct an acceleration lane and/or a left-turn lane on Waverly Road to permit safe egress from the site and ingress from the public roadway. Road construction and design shall be subject to approval by the Department of Public Works. The applicant shall pay for the cost of road construction. Condition No . 18 : The applicant shall retain a certified civil engineer to con- duct annual inspections and report to the Planning Division. Such reports are to attest to the applicant ' s compliance with activities authorized or required under this permit . Condition No. 19 : Use of the proposed truck scale shall be limited to customers in the process of hauling materials from the site. Condition No. 20: Site access to Waverly Road shall be located approximately 150 feet southerly of the existing entrance and shall be constructed with a 50-foot minimum radius and a 100-foot acce- leration and deceleration lane, also due to approval of the Department of Public Works. The use of other routes for access to the site is prohibited. Condition No. 21 : Prior to the commencement of excavation, the applicant shall submit for the approval by the decision-making body: a. A revised site plan that shows site access, location of structures, excavation phasing, project boundaries, paved surfaces, and elevations of excavation depths and final sur- faces. b. A revised excavation and rehabilitation schedule which details the locations, acreages, and cubic yardages to be excavated and rehabilitated, and the timetables and sequen- ces in which these operations shall be accomplished. c. A revised rehabilitation plan which incorporates the reseeding recommendations of the Soil Conservation Service or the Agricultural Extension Service.
