Appendix A Site Closure Request July 2,2010 Speedy Foods-Stockton Page 3 of 13 On January 18, 2005, Ground Zero personnel supervised the installation of vapor extraction well VW2 by V&W. On February 28, 2005, a Ground Zero technician supervised the destruction of the groundwater monitoring well MW2 in response to a letter dated February 15, 2005 sent by Effl) requesting destruction of the well to facilitate the Highway 99 Interchange Project being directed by CalTrans. A SollecoO thermal oxidizer soil vapor treatment system (VTS) was placed in operation on August 8, 2005 connected by sub-grade piping to two vapor extraction wells (VWl and VW2), a dual- phase extraction well (EW1), and two monitoring wells (MW3 and MW4). Details are discussed in Ground Zero's Quarterly .Monitoring Report Janamy through March 2005 dated May 2, 2005 and Quarterly Monitoring Report Judy through September 2005 dated November 22, 2005. Ground Zero submitted the Feasibility Study - Enhancement of Existing Remediation Activities dated September 25, 2007 which was approved by the EHD in a letter dated October 22, 2007. Ground Zero personnel supervised the installation of three groundwater monitoring wells between October 9 and 25, 2007 by V&W. V&W installed monitoring wells MW19D (D Zone aquifer), MW18C (C Zone aquifer) and MW17B (B Zone aquifer). Dewey Data Inc. of Stockton, California performed an e-log of MW19D. On November 7, 2007, Del-Tech developed the new monitoring wells. On November 2, 2007, Mr. Vasquez supervised the advancement of two soil confirmation borings (CBI and CB2) by V&W. The confirmation borings were advanced in the location of the fornier USTs. Details are discussed in Ground Zero's Monitoring Well Installation and Soil Investigation Report-Fall 2007, November 28, 2007. On December 4, 2007, Ground Zero personnel supervised the installation of groundwater extraction well EW3 by V&W. The well was surveyed by Morrow Surveying on December 1, 2007. On January 17, 2008, Del-Tech developed well EW3. Details are discussed in Ground Zero's Quarterly Groundwater Monitoring and Treatment System Operational Report October through December 2007 dated February 6, 2008 and Quarterly Groundwater Monitoriuug, Groundwater Treatment System Operation and Effectiveness, Soil Vapor Extraction System Rebound Test, and Additional Site Work Report-January through March 2008 dated April 7, 2008. Site closure was requested in Ground Zero's Site Closure Request and Senui-Annual Groundwater Monitoriuug Report- Winteu/Spring 2010, dated March 24, 2010. Ina letter dated June 1, 2010, EHD directed the submittal of a work plan to begin destroying the wells no longer necessary for the site investigation. Ground Zero submitted the Groundwater Monitoring Well Destruction Workplan dated June 9, 2010. The June 1, 2010 letter also request additional information be submitted following the requirements of the Tri-Regional Guidelines Appendix A. This document provides the additional information required to continue the review for low-risk closure.
