Appendix A Site Closure Request July 2,2010 Speedy Foods-Stockton Page 2 of 13 Between October 16 and October 18, 2000, Ground Zero personnel supervised Spectrum during the installation of four groundwater monitoring wells (MW5 through MW8) in the shallow water bearing zone (A Zone aquifer). Between October 17 and October 23, 2000 Spectrum installed groundwater monitoring well (MW9B) in the intermediate 120-foot water bearing zone (B Zone aquifer). Wellhead elevations of all site monitoring wells were surveyed, and the newly installed monitoring wells were developed between October 23 and 25, 2000. Details are discussed in Ground Zero's Additional Subscuface Investigation Scmunay Report dated February 28, 2001. In early February 2002, a new water supply well was placed into service after passing required water quality tests and receiving approval from the San Joaquin County Enviromnental Health Department (EHD). The old well was subsequently destroyed by Clark Well Inc. wider the supervision of Ms. Ellsaesser and Mr. Willie Ng of EHD. Between October 6 and October 8, 2003, Ground Zero personnel supervised V&W Drilling, Inc. (V&W) during the installation of three groundwater monitoring wells (MW 10, MW 11, and MW 12) in the A Zone aquifer and one vapor extraction well (VW 1). Between October 8 and October 16, 2003 V&W installed monitoring wells MW13C (C Zone aquifer) and MW14B (B Zone aquifer). The groundwater monitoring wells were developed between October 17 and October 23, 2003. On October 28, 2003 Morrow Surveying surveyed the newly installed monitoring wells. On October 22, 2003 Del-Tech Geotechnical Support Services (Del-Tech) performed short-term vapor extraction tests on wells MW 1 and VW 1 using an internal combustion engine equipped with a catalytic converter; instrumentation for measuring air flow, air pressure/temperature, and volatile organic compound concentrations; and PVC piping and fittings for wellhead connections. Details are discussed in Ground Zero's Monitoring Well Installation Report dated May 20, 2004. On October 29, 2003, groundwater samples were collected from well MW1 to satisfy California Regional Water Quality Control Board, Central Valley Region (RWQCB) Wastewater Discharge Requirements (WDRs) General Order No. R5-2003-004412, which was issued by the RWQCB in a letter dated November 5, 2003. The groundwater treatment system (GWTS) was put into operation on November 5, 2003. On June 30, 2004 and July 1, 2004 Ground Zero personnel supervised V&W during the installation of monitoring wells MW 15 and MW 16. The newly installed wells were developed on July 6, 2004. On July 20, 2004, Ground Zero personnel supervised the installation of mon 1 toring/ex tract Ion well EW2. The newly installed well was developed on August 3, 2004 and brought on-line oil November 24, 2004.
