5.0 INTERVIEWS <br /> 5.1 Interview with Client <br /> Mr. Foroutan was interviewed by telephone on May 3, 2018. At this time, he <br /> indicated that he has owned the Site since June of 2017. Mr. Foroutan stated <br /> that all three homes and the shed on the Site were built in the year 2004 or later. <br /> The homes receive municipal water. He indicated that the house on Stockton <br /> Street shares a septic system with the smaller house; the house on Alice Avenue <br /> has its own septic system. Mr. Foroutan added that the small house is vacant, <br /> and he only uses it occasionally when visiting the area. <br /> He stated that he was not aware of any fuel tanks, chemical storage, automotive <br /> maintenance, dumping of hazardous materials, or other environmental issues <br /> with the potential to impact the Site. <br /> 6.0 SITE DOCUMENTS <br /> 6.1 _Environmental Health Department Records <br /> Hazardous Materials Records <br /> The San Joaquin County Environmental Health Department was contacted <br /> regarding the Site addresses of 8505 West Stockton Street and 8500 West Alice <br /> Avenue, Thornton. According to Dom Martorella, no records related to <br /> hazardous materials are on file (personal communication, May 19, 2018). <br /> Well & Septic Permits <br /> Live Oak reviewed the San Joaquin County Environmental Health Department <br /> well and septic permit files for the Site addresses. Three permits were identified.. <br /> • May 2004 permit for new septic system at residence (8505 W. Stockton). <br /> The system utilized a 1,600-gallon septic tank and four 100-foot leach <br /> lines. <br /> • March 2007 permit for replacement filter bed at residential septic system <br /> (8505 W. Stockton St). The permit notes that "existing leach field is being <br /> abandoned and relocated to allow for 2nd unit dwelling." The new system <br /> utilized a 30 by 50-foot filter bed. <br /> • March 2007 permit for new septic system at residence. The system <br /> utilized a 24 by 50-foot filter bed (8500 Alice). <br /> LOGE 1822 4 <br />
