MANTECA August 25, 2016 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> At the time of inspection, it could not be demonstrated (not necessarily documented) that employees who handle <br /> hazardous waste were properly trained. Lourdes De Los Santos indicated that no hazardous waste specific training <br /> is being provided, she was unclear at the time of inspection as to the the hazardous waste labeling requirements. The <br /> generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br /> handling and emergency procedures. Provide proof of training to the EHD for employees whose responsibilities <br /> include hazardous waste. <br /> This is a Class II violation. <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> 1) Copies of the following uniform manifests for the disposal of hazardous waste were not found on site: <br /> 004495177SKS, 004494998SKS,004494697SKS, and 002829217GBF. Hazardous waste generators shall retain <br /> copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting <br /> procedure on site for three years and have them readily available for review. Immediately locate a copy of all <br /> manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. <br /> 2) Manifest numbers 002829302GBF, 002829508GBF,003039633GBF, 003040163GBF,003040342GBF, and <br /> 003039941 GBF were found without a signed off copy from the destination facility. Hazardous waste generators shall <br /> retain copies of all manifests signed off by the destination facility on site for three years and have them readily <br /> available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the <br /> owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste <br /> was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some <br /> indication that the generator has not received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC.
