MANTECA _ <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item # Remarks <br /> 111 CCR 66262.42(a)(c)(d) Failed to comply with uniform hazardous waste manifest exception requirements. <br /> Manifest numbers 002829302GBF,002829508GBF,003039633GBF,003040163GBF, 003040342GBF, and <br /> 003039941 GBF were found without a signed off copy from the destination facility. Hazardous waste generators shall <br /> retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The <br /> signed off copy shall be retained and made readily available for review for three years. If the generator did not <br /> receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the <br /> generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the <br /> generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received <br /> confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit <br /> to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. <br /> This is a Class II violation. <br /> 403 CCR 66262.34(4)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste. <br /> Two 55-gallon containers of hazardous waste were partially open; these containers had lids but were missing <br /> container ring clamps, and couldn't be re-closed at the time of inspection. All hazardous waste containers shall be <br /> closed at all times except when adding or removing waste. Immediately close these containers and ensure all <br /> hazardous waste containers are closed when not adding or removing waste. Submit proof of correction to the EHD. <br /> This is a Class II violation. <br /> 601 CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days. <br /> One black metal 55-gallon container (1/2 full) had a hazardous waste label which was lacking an accumulation start <br /> date. One blue plastic 55-gallon container had a hazardous waste label which was also lacking an accumulation start <br /> date. One small container (20-30 gallons in capacity) had an accumulation start date of 11-3-2015. A total of four <br /> 55-gallon containers and one 20-30 gallon containers of hazardous waste were onsite at the time of inspection. <br /> Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored <br /> on site at any time may store waste on site up to 180 days. Immediately contact a licensed hazardous waste hauler <br /> to dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br /> This is a Class II violation. <br /> Page 5 of 6 <br />
